Partnership HealthPlan of California (PHC)The Partnership HealthPlan of California began operations on May 1, 1994, and is a public-private organization designed to provide a cost-effective method of health care delivery to over 86,000 Medi-Cal and Healthy Kids Program members in Napa, Solano, Sonoma and Yolo Counties in Northern California. Approximately 51,000 PHC members are Solano County residents. PHC's goals are to improve access, quality and cost effectiveness, through a managed care system. The HealthPlan links members with a medical home and has been successful in reducing the inappropriate use of emergency departments, providing the right level of inpatient care, developing innovative case management programs and providing more services locally.

Many low-income adults cannot afford the cost of the medications that are essential to managing their health and chronic conditions. Most pharmaceutical companies have patient assistance programs which make selected medications, particularly for chronic conditions, available to low income residents who do not qualify for publicly-funded insurance programs.
In 2003, the Coalition created a countywide Pharmacy Assistance Program to assist these adults secure free medications. The PAP coordinator provides on-site services at the County Family Health Centers, La Clinica de la Raza in Vallejo, and Community Medical Centers in Dixon and Vacaville to assist residents in completing applications to receive required pharmaceuticals. PAP assists approximately 500 residents secure in excess of $1 million in essential medications annually.
Solano County African American Disparities elimination ProjectBeginning in 2004, the Coalition refocused its energy to eliminate racial and ethnic disparities in health status. Based on its initial research, the Coalition concentrated on addressing the health disparities that adversely affect African American residents of Solano County. The Coalition, in partnership with the African American Community, determined that this work should be guided by a comprehensive, strategic plan. In March 2005, the Coalition convened a Disparities Elimination Planning committee comprised of community leaders, Coalition Directors and representatives from partner organizations. this Committee has developed a community-based strategic plan that was endorsed by the Solano Coalition for Better Health Board of Director and presented to member of the Solano County community on March 26, 2007.
