Community Health Education

Coalition leaders are featured speakers at community events.  The Coalition utilizes a variety of strategies to inform and educate the community about important health issues including health education in classrooms, at events and on an individual basis.  Coalition issues and "stories" are covered by print and electronic media, and articles on key health topics appear in local daily newspapers and other publications.

Healthy Kids Solano

The Coalition is committed to providing health coverage for all children.  Many uninsured children qualify for publicly sponsored health insurance.  Healthy Kids Solano was created to provide health insurance for the children not eligible for publicly funded programs.

Healthy Kids Solano provides comprehensive medical, dental and vision benefits at an affordable cost of $1,200 per child per year.  The Coalition has secured funding from Solano County, First 5 Solano, First Five California, United Way, The California Endowment, Blue Shield, foundations, local businesses, service clubs and individuals to cover the costs.

Partnership HealthPlan of California (PHC)

The Partnership HealthPlan of California began operations on May 1, 1994, and is a public-private organization designed to provide a cost-effective method of health care delivery to over 86,000 Medi-Cal and Healthy Kids Program members in Napa, Solano, Sonoma and Yolo Counties in Northern California. Approximately 51,000 PHC members are Solano County residents. PHC's goals are to improve access, quality and cost effectiveness, through a managed care system.

The HealthPlan links members with a medical home and has been successful in reducing the inappropriate  use of emergency departments, providing the right level of inpatient care, developing innovative case management programs and providing more services locally.

Pharmacy Assistance Program (PAP)

Many low-income adults cannot afford the cost of the medications that are essential to managing their health and chronic conditions.  Most pharmaceutical companies have patient assistance programs which make selected medications, particularly for chronic conditions, available to low income residents who do not qualify for publicly-funded insurance programs.

In 2003, the Coalition created a countywide Pharmacy Assistance Program to assist these adults secure free medications.  The PAP coordinator provides on-site services at the County Family Health Centers, La Clinica de la Raza in Vallejo, and Community Medical Centers in Dixon and Vacaville to assist residents in completing applications to receive required pharmaceuticals.  PAP assists approximately 500 residents secure in excess of $1 million in essential medications annually.

Solano County African American Disparities elimination Project

Beginning in 2004, the Coalition refocused its energy to eliminate racial and ethnic disparities in health status.  Based on its initial research, the Coalition concentrated on addressing the health disparities that adversely affect African American residents of Solano County.  The Coalition, in partnership with the African American Community, determined that this work should be guided by a comprehensive, strategic plan.

In March 2005, the Coalition convened a Disparities Elimination Planning committee comprised of community leaders, Coalition Directors and representatives from partner organizations.  this Committee has developed a community-based strategic plan that was endorsed by the Solano Coalition for Better Health Board of Director and presented to member of the Solano County community on March 26, 2007.

The Coalition supports the work of a 25 members disparities Elimination Strategic Planning Committee which is charged with assisting the Coalition and its partners to eliminate disparities in health outcomes among Solano county's African American residents.

Disparities Community Leaders Work Group
The Coalition supp0rts a 20 member Community Leaders Disparities Elimination Work Group which develops recommendations for activities that will increase awareness of the health disparities in the African American Community by partnering with other leadership groups to provide information and promote healthy behavior.

Disparities Physicians Advisory CommitteeThe Coaltion supports a 15 member Physicians Advisory Committee who are contributing their time to advise the Disparities Elimmination Planning Committee.  The Advisory Committee provides recommendations to enhance the quality of care for African Americans and recommend educational opportunities for providers.

Solano Kids Insurance Program (SKIP)

The Solano Kids Insurance Program (SKIP) was implemented in 1998 to link uninsured kids with health care coverage. SKIP is a centralized one-stop health insurance resource for families and provides the following services :

  • Provides information about health care services and insurance options

  • Assists families to enroll in  Medi-Cal, Healthy  Families, Kaiser Child Health Plan, Healthy Kids Solano, and other Health care services

  • Assists families to gain access to health care services

The Community Clinic Alliance

The Community Clinic Alliance is comprised of CEOs and managers of community and county clinics. The Alliance advocates for medically underserved and often uninsured residents and raises awareness of their  needs with the Coalition and community.

Health Access Strategic Planning Committee

The Coalition supports the work of a 25 member health access strategic planning committee comprised of leaders from major health care providers and organizations.  Five work groups are dedicated to achieving the following objectives from the Solano County Health Access Strategic Plan: 2005-2008:

  • Enrolling and retaining all eligible children and adults in available public or other subsidized plans or health program(s)

  • Expanding integrated behavioral health services at primary care sites

  • Increasing the availability of dental services

  • Reducing the inappropriate use of the healthcare system by those who frequently use that system

  • Addressing beliefs, attitudes, knowledge and experiences that negatively impact appropriate utilization of health services
